TRYOUTS WILL BE HELD: April 23-27, 2012 @ UHS
Tryout Packets (with more specific information) are available at UHS & VJH Front OfficesTo be eligible to tryout you must:
1. Meet GPA Standard: Prior Grading trimester/qtr: 2.5 or above.
2. Not have any F's grades or U's.
3. Be Student at UHS for 2012-13 and be enrolled in four classes or equivalent. (Four classes currently does not include released time.)
Required Paperwork which is due the first day of auditions:
1. Completed USD Drug Consent Form and submit results from a drug test performed at Toxicology Testing Services (35 W. 100 S.) This test is paid for by UHS.
2. Completed USD Athletic participation Consent and Liability Disclosure. Include all signatures by student and parent or legal guardian.
3. Completed PHYSICAL FORM either A or B.
4. UHSAA Tryout Checklist - signed by student and parent/guardian.
5. U'ette Application - to be completed by potential U'ette.
6. Copy of candidate's 2nd Trimester or 3rd Quarter REPORT CARD. (Not a transcript--the actual report card.)
